- Sewer - a sewer main has to be diverted through a siphon to make way for a Metro railway. The siphon blocks the escape of dangerous fumes, an explosion occurs, and hundreds are killed.
- Fume siphon over tunnel would have allows fumes to be detected downstream, and to disapate.
- Quintinshill (1915) - signalman on a railway forgets about a train, albeit while rorting the train register book, and collisions and fire kill 200+.
- More tolerance about shift change hours would have avoided distraction of rorting books.
- Track circuits could have held signals at red.
Help identifying the TV documentary involved would be appreciated.
