Premature ejaculation is most often defined as the act of ejaculating during sexual intercourse before both partners are ready, or an ejaculation over which a man has no voluntary control. Historically, however, it has been defined as occurring when the man ejaculates within two minutes of penetration more than half the times a couple have intercourse. A survey by Alfred Kinsey in the 1950's demonstrated that three quarters of men fell into this category. To this extent, therefore, premature ejaculation can be considered normal, not pathological.

By contrast, rapid ejaculation is a term used when ejaculation soon follows penetration, but the partners are both satisfied with their sexual experience. By definition, therefore, premature ejaculation implies that there is dissatisfaction that sexual intercourse has not been prolonged.

Although a few men tend to ejaculate rapidly as a response to anxiety or stress, most men who ejaculate rapidly have done so consistently from their earliest experiences with a partner.

The cause of premature ejaculation remains unclear, though some authorities consider that anxiety, anger or fear are precursors to rapid ejaculation. Others have speculated that early experiences with masturbation condition a boy to ejaculate rapidly, and that this pattern of sexual responsiveness is then maintained in adult sexual experiences.

With the correct motivation, premature ejaculation can usually be treated and totally cured. There are several treatments available. The most common are desensitization creams or condoms, containing a local anesthetic, although these have the disadvantage that they may reduce sexual pleasure for both partners. Exercises designed to train the body to respond with lower sexual arousal to sexual stimuli are effectve in the majority or men, and must be considered the best cure.
